ACE – Consent & School Policies

1. Participation Consent

By submitting this form, I acknowledge that I am voluntarily enrolling my child/myself in ACE programs. I understand that ACE instructors will provide learning activities that may include academic instruction, arts-based activities, cultural workshops, and other supervised educational experiences. I authorize my child/myself to participate fully in all program activities unless otherwise communicated in writing.

2. Safety & Conduct Policy

ACE maintains a safe, supportive learning environment. Students are expected to:

• Treat peers, staff, and property with respect

• Follow classroom guidelines and teacher instructions

• Maintain appropriate behavior in all in-person and online sessions

ACE reserves the right to remove a student from an activity or program if behavior compromises safety or the learning environment. Parents/guardians will always be notified of concerns.

3. Attendance & Scheduling Policy

Regular attendance supports learning and program continuity.
I understand that:

• ACE must be informed in advance of any planned absences.

• Missed sessions may not always be rescheduled, depending on instructor availability.

• Persistent absences may affect progress or program continuation.

4. Media Release (Optional but Recommended)

I grant permission for ACE to take photos or videos during classes, workshops, or events for documentation and promotional purposes (website, brochures, social media).
No names or identifying information will be shared without additional consent.
(Parents may opt out by selecting No on the form.)

5. Communication Policy

ACE communicates with parents and students through email, text, or phone.
By enrolling, I agree to receive:

• Class updates

• Schedule reminders

• Program-related announcements

ACE will not share my contact information with any third party.

6. Liability Waiver

I understand that ACE provides structured programs with safety protocols in place.
By enrolling, I agree that:

• I will not hold ACE, its instructors, or staff liable for accidental injuries that may occur during normal participation.

• I am responsible for notifying ACE of any medical, learning, or behavioral needs that may require accommodations.

7. Refund & Cancellation Policy

I acknowledge that:

• Program fees (if applicable) may be refundable only within ACE’s posted refund window.

• Once programs begin, refunds or credits are not guaranteed.

• ACE may cancel or reschedule sessions due to emergencies, safety concerns, or instructor availability.

8. Academic Integrity & Learning Expectations

Students agree to:

• Engage actively and do their own work

• Maintain respect for the learning process

• Use ACE resources appropriately

Parents/guardians agree to support the student’s learning goals and ACE’s instructional approach.

9. Technology & Online Learning Policy (for Online Programs)

For online classes, I agree that:

• The student will join sessions using appropriate language, conduct, and digital citizenship

• ACE is not responsible for technical issues on the participant’s side

• Sessions may not be recorded or distributed without permission

10. Cultural Respect Policy

ACE teaches art, culture, languages, and heritage topics.
By enrolling, I agree to:

• Show respect toward all cultural materials and practices

• Support a discrimination-free environment

• Uphold ACE’s mission of cultural appreciation and inclusion

11. Consent Confirmation

By checking the box on the form, I confirm that:

• I have read and understood ACE’s policies

• I agree to abide by these guidelines

• I give permission for my child/myself to participate in ACE programs
